Thank you for signing up to be an RSPCA animal hero. If you are under 18, please ask your parent/guardian to complete this form for you with your challenge details on page 1 and their details on page 2.

As a member of Team Animal you're making lives better for lots of animals - so thank you!

There's no fundraising target for your challenge, just raise as much as you can! If you raise £50, you will receive a digital certificate as a big thank you and if you raise £250, your very own subscription to our Animal action magazine!
